Dear patients,
We have instilled the following safety measures to ensure that both out patients and staff stay safe.
1) All patients will be required to wear a mask to their appointment. No exceptions
2) Temperatures will be taken upon entering the office. Anyone with a fever will be rescheduled
3) Patients will be required to fill out a COVID declaration form
4) Exam and pretesting areas will be cleaned/sanitized before and after each patient
5) We are scheduling one patient every 30 minutes to ensure proper social distancing protocols
6) To reduce in office wait time, patient registration forms can be found at the top of our website under the tab "patient forms"
